The Appraisal Forms
The following is a list of Appraisal forms by Hawk Appraisals:
Single Family Homes -
1004 - Uniform Residential Appraisal Report
This report form is designed to report an appraisal of a one-unit property or a one-unit property with an accessory unit, based on an interior and exterior inspection of the subject property. Includes comparable sales, cost and income approaches to valuation.
1004C - Manufactured Home Appraisal Report
This report form is designed to report an appraisal of a one-unit manufactured home; based on an interior and exterior inspection of the subject property. Includes comparable sales, cost and income approaches to valuation.
1004D - Appraisal Update and/or Completion Report
This report form is intended to provide the lender/client with an accurate update of a prior appraisal and/or to report a certification of completion. The appraiser must identify the service(s) provided by selecting the appropriate report type.
1004MC –Market Conditions Addendum
The Market Conditions Addendum is designed to enhance the understanding of the market trends and conclusions made by the appraiser. The Market Conditions Addendum is required with all one- to four-unit property appraisals. This requirement applies to all loans delivered to Fannie Mae.
2055 - Exterior-Only Inspection Residential Appraisal Report
This report form is designed to report an appraisal of a one-unit property or a one-unit property with an accessory unit; based on an exterior-only inspection of the subject property from the street. This report form is not designed to report an appraisal of a manufactured home or a unit in a condominium or cooperative project.
Multi-Family Homes -
1025 – Small Residential Income Property Appraisal Report
This report form is designed to report an appraisal of a two to four-unit property, based on an interior and exterior inspection of the subject property. Includes comparable sales, cost and income approaches to valuation.
216 – Operating Income Statement
The lender should use this form to determine the amount of operating income that can be used in evaluating the applicant's credit on applications for conventional mortgages that are secured by one-family investment properties and all two- to four-family income producing properties.
Land -
Land Appraisal Report – Vacant Land Appraisals
This report form is designed to report the value of vacant land property.

2000 - One-Unit Residential Appraisal Field Review Report
A lender may use this form to spot-check appraisal component of its quality control process for a single unit or single unit with accessory form. This appraisal field review report is intended to provide the lender/client with an opinion on the accuracy of the original appraisal report under review. This form provides at least an exterior from the street review of the subject.
2000A - Two- to Four-Unit Residential Appraisal Field Review Report
A lender may use this form to spot-check appraisal component of its quality control process for a two to four unit form. This appraisal field review report is intended to provide the lender/client with an opinion on the accuracy of the original appraisal report under review. This form provides at least an exterior from the street review of the subject.
